Tennessee’s prison chief is out. Department of Correction Commissioner Frank Strada resigned Saturday, days after the state failed to execute Christa Pike and left her fighting for her life on a ventilator instead.
The execution went wrong Wednesday at Riverbend Maximum Security Institution in Nashville. Pike, 50, was injected with two rounds of the lethal drug pentobarbital and did not die. Media witnesses said they could hear her snoring for about an hour after the drugs went in, and she was still alive when they were ushered out of the viewing room. She is unconscious and intubated while doctors work to clear the drug from her system.
Her lawyers say the state ignored warnings from her own doctors. They flagged her small veins and a blood disorder called thrombocytosis ahead of time, saying placing the IV lines would be difficult. The lawyers believe the drug seeped into the tissue around her veins instead of her bloodstream, and nobody on the execution team seemed to realize anything was going wrong.
Pike was set to become the first woman executed in Tennessee in 206 years. She has been on death row since 1996 for the 1995 torture and killing of 19-year-old Colleen Slemmer. Pike was 18 at the time.
Governor Bill Lee, who denied Pike clemency earlier, has now halted every execution left on the 2026 calendar and ordered an independent review of what happened. Strada said he still believes his department followed protocol, but stepping aside was in the best interest of the state while the review plays out. He had run the department since 2023, after coming over from Arizona where he was a top official during three lethal injections that drew heavy criticism of their own.
Pike’s lawyers called the resignation justified but said it does nothing for her right now. “What happened to her Wednesday reflects a systemic failure that goes far beyond any one person,” they said.
